UK State Pension Life Certificate in Ontario: How a Notary Public Can Help

If you receive a UK State Pension while living in Ontario or elsewhere outside the United Kingdom, you may occasionally be asked by the UK Department for Work and Pensions (DWP) to complete a Life Certificate.

Many pensioners are unsure who can witness the form or where to have it completed. Fortunately, if you live in Ontario, a Notary Public can usually act as an authorized witness.

What Is a UK Life Certificate?

A Life Certificate is a document issued by the UK Department for Work and Pensions (DWP) to verify that a person receiving a UK State Pension is still alive and remains eligible to receive pension payments.

The DWP does not require every pensioner to complete one every year. Instead, Life Certificates are sent periodically to selected pensioners living outside the UK.

Important: Only complete and return a Life Certificate if the DWP has specifically asked you to do so.

Who Can Witness a UK Life Certificate?

According to the UK Department for Work and Pensions, the witness must:

  • Not be related to you by birth or marriage.

  • Not live at the same address as you.

The DWP accepts many different professionals as witnesses, including:

  • A Notary Public

  • A lawyer, barrister, solicitor, or advocate

  • A Commissioner for Oaths

  • A doctor, dentist, nurse, pharmacist, or optician

  • A teacher or lecturer

  • A police officer

  • A bank officer authorized to sign documents

  • A civil servant or government employee

  • A Justice of the Peace or magistrate

  • A minister of a recognized religion

  • Certain other qualified professionals

As an Ontario Notary Public, we can witness your Life Certificate in accordance with the DWP's published guidance.

What Should You Bring?

For your appointment, please bring:

  • The original Life Certificate sent to you by the UK Department for Work and Pensions.

  • Valid government-issued photo identification (such as a passport, driver's licence, or Ontario Photo Card).


How Do I Return the Form?

After your Life Certificate has been witnessed, you are responsible for mailing the completed original to the address provided by the Department for Work and Pensions.
