Preparation Checklist for Your Appointment

A valid, government-issued photo ID is mandatory (e.g., driver’s license, passport, or official ID card). Expired ID and student IDs are not accepted.

The name on your photo ID must exactly match the name on your documents.

All forms must be fully completed before arrival, but do not sign until in the presence of the notary.

Fees are calculated per signature and/or seal, not per document.

A waiver (which will be sent to you upon booking/available in office) must be fully completed before the service begins.
