What Is the Custodianship Declaration (IMM 5646E)?
The Custodianship Declaration is an official IRCC form used when a minor (under 18 years old) will study in Canada without a parent or legal guardian present.
The form confirms that:
A responsible adult in Canada agrees to act as the child’s custodian
The custodian accepts responsibility for the child’s care and supervision
The parent(s) or legal guardian(s) authorize the arrangement
This document is often required as part of a minor’s study permit application.
Who Needs to Sign the IMM 5646E Form?
The form contains two separate declarations:
1. Declaration by the Custodian (In Canada)
The custodian (the adult responsible for the child while in Canada) must sign their section in front of a notary public or commissioner for oaths in Ontario.
2. Declaration by Parent(s) or Legal Guardian(s)
The parent(s) or legal guardian(s) complete and sign their section in their home country before a local notary or authorized official.
These sections are usually completed separately and do not need to be signed at the same time.

Use the most recent IMM 5646E form from IRCC.
